The causes of tired-looking eyes
In order to find the right treatment method, the exact cause of the visual impairment must first be determined. There are three main problem areas on the lower eyelids:
- Loss of volume and dark circles under the eyes: With age, the midface loses fat and bone volume. This leads to the formation of a deep furrow between the lower eyelid and the cheek – known as the tear trough. This indentation casts a shadow that is perceived as a dark circle under the eye. In many cases, there is no excess skin.
- Eye bags (fat protrusion): This is fatty tissue that normally surrounds the eye and protrudes due to an age-related slackening of the membrane. This protruding fat forms visible swelling or bumps under the eye that remain regardless of tiredness.
- Skin and muscle slackening: Over time, the skin and the underlying orbicularis oculi muscle lose elasticity. This leads to fine wrinkles, crepe-like skin texture and significant excess skin, often in combination with bags under the eyes.
The method of choice depends largely on which of these components are most prominent in your case.
Removing dark circles under the eyes with fillers: gentle correction
If the main cause of tired-looking eyes is loss of volume and the resulting tear troughs, treatment with fillers is often the ideal solution.
This is a minimally invasive injection treatment, usually with highly cross-linked hyaluronic acid. Hyaluronic acid is a natural component of the skin and has the ability to bind water and build volume.
Using a fine cannula (often blunt to minimise the risk of bruising and injury), the filler is injected precisely into the deep tear trough. The aim is to lift the depression and bring it to the same level as the surrounding skin. This eliminates the shadow cast and makes the dark circles under the eyes disappear.
The method is indicated for patients with pronounced dark circles under the eyes due to volume loss (tear trough) and relatively good skin elasticity. It is the ideal solution for younger patients or patients with mild to moderate volume loss. The results are immediately visible and the treatment often takes only 15 to 30 minutes. Depending on the material used and the patient, the results last between 9 and 18 months.

Lower eyelid lift: The surgical approach
If the problems go beyond a simple lack of volume and there is a significant excess of skin, muscle tissue or fat pads (eye bags), then a lower eyelid lift is the only permanent and effective solution.
Lower eyelid lift (surgical blepharoplasty) is a surgical procedure to correct excess skin and fat in the lower eyelid area.
There are two main approaches that LIVION uses, depending on the findings:
- Transcutaneous (through the skin): An incision is made directly below the lash line. This allows excess skin and/or fatty tissue to be removed and the orbicularis oculi muscle to be tightened. This approach is indicated for patients with significant excess skin.
- Transconjunctival (via the conjunctiva): Here, the incision is made on the inside of the lower eyelid (conjunctiva). This approach is preferred when the primary aim is to remove or reposition the tear sacs (fat pads) and there is minimal or no excess skin. The advantage is that there is no visible scar on the outside.
