Breast lift and augmentation - does the combination make sense?

Many women become dissatisfied with their breasts at some point. They often wonder whether combining a breast lift with breast augmentation is a good idea. In many cases, the answer is yes: if you desire fuller and simultaneously firmer breasts, this combination can be highly effective. However, several factors must be considered — your starting point, anatomical conditions, desired outcome, and the surgical approach.

At LIVION in Hamburg, we offer both breast lift procedures (“mastopexy”) as well as breast augmentation using implants or fat transfer. Still, combining the two procedures is not always necessary or suitable for every patient. In this article, you will learn how these treatments work, the advantages they offer, what to consider, and which method may be the right choice for you.

Breast lift at LIVION - for Sagging Breasts

A breast lift is indicated when your breasts have lost shape due to pregnancy, breastfeeding, significant weight loss, or natural aging, resulting in sagging that is visible or palpable. At LIVION, this procedure is performed under general anesthesia. The surgery usually lasts two to three hours and involves removing excess skin, repositioning the nipple, and tightening the remaining breast tissue. Typically, patients stay one night in the clinic. Most are socially active again after seven to ten days, and sports can generally be resumed after about six weeks. LIVION emphasizes scar-sparing techniques and individualized preoperative consultation.

A breast lift alone does not significantly increase breast volume – it primarily lifts and shapes. Many women therefore ask whether an additional augmentation can enhance the aesthetic result.

 

Breast augmentation with implants - an established method for increasing volume

At LIVION, you have the option of breast augmentation with implants. This procedure is also performed under general anesthesia. The operation typically lasts around two hours, and an overnight hospital stay is required. LIVION uses high-quality, certified implants and offers round, anatomical, and teardrop-shaped options. Round implants create a more even increase in volume, while the teardrop-shaped option looks more natural and follows the natural contours of the breast.

The procedure at LIVION typically involves the following steps: The incision is usually made in the inframammary fold – alternatively, via the armpit or areola. The implant is usually placed under the pectoral muscle, the wound is sutured in multiple layers, and a compression bra is applied. After the surgery, you will receive all the important information regarding aftercare. To ensure stability, the bra should be worn day and night for approximately six to eight weeks; during this time, saunas, swimming, and sports should be avoided.

Breast augmentation with implants is particularly suitable if you desire significantly greater volume or have little existing breast tissue. Combined with a breast lift, the result can be harmonious and long-lasting.

 

Breast augmentation with autologous fat transfer - natural, but with limitations

Another method offered at LIVION is breast augmentation with autologous fat transfer. This procedure is also performed under general anesthesia. The operation takes approximately two to three hours, and an overnight stay in the clinic is required afterward. Fat is harvested from areas such as the abdomen, hips, or thighs, processed, and then transplanted into the breasts.

A key goal of this method is to increase breast volume by approximately half to a full cup size. An important prerequisite is the presence of sufficient fat cells. Furthermore, the engraftment rate of the transplanted fat cells is typically around 60 to 80 percent, meaning that some of the fat is always absorbed.

The autologous fat transfer method has clear advantages: it uses the patient's own tissue, avoids foreign materials, and also shapes the donor area. However, the increase in volume is limited, the shapeability differs from that of implants, and a second procedure may be necessary. This method is more suitable for patients who desire a moderate augmentation in addition to a lift and place great value on a natural appearance.

 

Why the combination of tightening and augmentation is often beneficial

If you want to firm up your breasts and also desire a larger bust, a combination of breast lift and augmentation can be beneficial. Augmentation alone, without a lift, can lead to sagging skin, unstable results, or an imperfect shape. Conversely, a lift without volume enhancement may not achieve the desired volume.

The combination allows the following:

  • Breast tightening through removal of excess skin and repositioning of the nipple.
  • Volume augmentation through implants or autologous fat, depending on preference and initial situation.
  • Harmonious shape and contour, more stable breast position and long-term aesthetic effect.

When does the combination make less sense?

Not every patient automatically benefits from a combination of breast lift and augmentation. There are cases where a single procedure is more appropriate: If the breasts sag but have sufficient volume, a lift alone is enough. Or, if skin quality is severely compromised and you have little breast tissue and fat reserves, augmentation with implants alone may be more suitable. Even in cases where your desired size is only slightly larger than your current size, fat transfer alone may suffice.
